Student Health Services Forms

It Is The Law: The Pennsylvania State Legislature passed a meningitis vaccination law in 2002 pertaining to students residing in residence halls. This legislation requires that all students residing in a residence hall in Pennsylvania be vaccinated for meningococcal meningitis before moving onto a college campus unless they have signed a waiver after receiving information about the disease and the benefits of the vaccine.

Please use The Student Health Portal for all other needs.

The Student Health Services staff's work is based on a comprehensive wellness philosophy.

Medical services, health information, and counseling are available to all students. Primary care (assessment and treatment) and referrals to community physicians and to campus and community resources for health education and counseling are provided. Strict professional privacy and confidentiality standards are maintained.

Patient's Bill of Rights

  • To considerate and respectful care

Resident students who do not submit the completed forms may be required to live off campus. (Approved by the Board of Trustees 2000).

HIPAA Law

For information on the HIPAA Law please visit the U.S. Department of Health and Human Services.

Medical Records Destruction

Students' medical records or any part thereof shall be destroyed by shredding, or other effective methods in keeping with the confidential nature of its contents. Destruction of such records must be made in the ordinary course of business.

Student Health Services is responsible for ensuring that the records have been properly destroyed.

Medical Records Retention

Pennsylvania rules and regulations pertaining to patients medical records, 49 Pa. Code § 16.95(e) (2008) and 28 Pa. Code § 115.23 (2008). Pennsylvania law requires physicians and hospitals to retain patient medical records for seven years from the last date of service or discharge. Records of minors must be kept for one year past the age of majority by physicians, and seven years past the age of majority by hospitals..

The Marywood University Student Health Services Policy is as follows:

  • Student Health Services does maintain health records for the duration of the student’s enrollment and for seven years beyond the date of graduation.  Records older than seven years are not available or retrievable.
  • Students who take a Leave of Absence: records shall be kept on file for seven years after the student either graduates or withdraws.
  • Students who Apply and Do Not Attend: records shall be kept on file for one year.

Medical Records Security

During hours of operation, Student Health Services is adequately staffed to safeguard records from unauthorized access.

After hours of operation, or when the department is unattended, medical records are secured under lock and key allowing only authorized personnel to the records.

COVID-19 FAQs

What is Marywood’s COVID-19 vaccination policy? Marywood University does not require all students to show proof of COVID-19 vaccination or a negative COVID-19 viral test before they arrive on campus. Please be aware that certain academic programs have contracts with hospitals, clinics, schools, private businesses, etc. that may require proof of COVID-19 vaccination in order for students to complete their clinical rotations, internships/externships, etc. in these facilities. It is recommended that you check with your respective program for any specific requirements.

How do I communicate my vaccination and booster status to the University? Students will upload proof of their COVID-19 vaccination and booster record to their secure Student Health Portal.

Will COVID-19 Testing be available on campus? Marywood University’s Student Health Services (SHS) will continue to offer COVID-19 testing for students who are exhibiting signs and symptoms of COVID-like illness and for those with a known exposure. All testing is done by appointment. Appointments can be made by calling SHS at 570-348-6249

Will COVID-19 vaccines be available on campus? SHS will continue to collaborate with our community partners to coordinate vaccine clinics both on-site and off-site to ensure vaccine availability.

Will masks be required anywhere on campus? There is no campus-wide mask mandate although we respect and support anyone wishing to wear a face covering at any time. Students reporting to SHS with signs and symptoms of respiratory illness will be asked to wear a mask upon entering the clinic. Masks will be available upon entering the student health center for anyone who needs or wishes to wear one.

What are the quarantine and isolation procedures for students?

Isolation Protocols for Students:

All students who test positive will be required to isolate at home, regardless of vaccination status.

No Symptoms – Isolate for 5 days from the date of the positive test.

• You can return to campus on day 6 AND wear a mask through day 10 if:

• You do not develop symptoms.

Symptomatic – Isolate for 5 days from your first symptom.

• You can return to campus on day 6 AND wear a mask through day 10 if:

• You are fever-free for at least 24 hours;

• AND Your symptoms have improved.

Protocols for close contacts of COVID + individuals:

Note: According to the CDC: “The date of your exposure is considered day 0. Day 1 is the first full day after your last contact with a person who has had COVID-19.”

All students who are considered a close contact to a known positive case will be required to follow one of the following protocols, depending on the presence of symptoms:

No Symptoms – No quarantine is necessary, but you MUST do the following:

  • Wear a mask as soon as you find out you were exposed- Start counting from Day 1
  • Day 0 is the day of your last exposure to someone with COVID-19; Day 1 is the first full day after your last exposure
  • Continue precautions for 10 full days and watch for symptoms
  • Testing is recommended at least 5 full days after your exposure even if you don’t develop symptoms.

If symptoms present you MUST do the following:

  • Isolate Immediately
  • Get tested and stay home until you know the result
  • If your test result is positive, follow the isolation guidance above

Will there be academic accommodations for COVID related absence? To ensure proper communication with your faculty related to absence, students are strongly encouraged to communicate with faculty in a timely manner to ensure they are keeping up with coursework and to articulate the inability to attend any face to face component of class/clinical during an isolation period.

Will the University be communicating COVID-19 updates to the campus community? Yes. The frequency of updates will fluctuate throughout the academic year.

Marywood University to Hold Community Stalking Awareness Campaign
January Is National Stalking Awareness Month
Scranton, PA (January 16, 2024) — January 2024 marks the 20th National Stalking
Awareness Month, an annual call to action to recognize and respond to the serious crime
      of stalking.
Stalking is defined as a pattern of behavior directed at a specific person that causes fear.
Many stalking victims experience being followed, approached, monitored and/or
threatened—including through various forms of technology. Victims and survivors often
suffer anxiety, social dysfunction, and severe depression as a result of their victimization,
and many lose time from work and/or move.
As a part of the Department of Justice Grant, Project SHARE, Marywood’s prevention
education efforts of the grant will be supporting a Stalking Awareness Week, January 22-
26, 2024. The events scheduled are as follows:
      ? January 22 - Sweet Information Table, Learning Commons, 11 a.m.- 2 p.m.:
This will be a tabling event where students can pick up information (flyers,
brochures, etc) about Stalking Awareness on a college campus.
? January 23 - Stalking In the Media Presentation, Upper Main, Nazareth Student
Center, 5-6 p.m.: This event is a presentation about how the media portrays
      stalking behaviors.
      ? January 25, 2024 - Stalking Awareness Presentation, given by the Women's
Resource Center (WRC), Swartz Center, from 3-4 p.m.: WRC will give a
