Help Desk - Equipment Request Form

PLEASE NOTE:

Equipment must be requested AT LEAST one week in advance or we cannot guarantee its availability.

Microphones MUST be requested for conference rooms.

Microphones are NOT available for classrooms.

Our technicians will assist in the set up of the equipment but may not be available at the time of, or during an event.

Students must present an Authorization Form, signed by a faculty member, before they borrow equipment.
