Preceptor Agreement - print out for a signature Preceptor resume - Required as per CADE standards. Memorandum of Agreement / Contract -due after acceptance  Professional Staff Form - only for currently enrolled interns Registration Registration Form - The form is under the Registrar's Office Heading. This form is to be used for registration for courses once admitted into the program.
