SITE SUPERVISOR EVALUATION FORM
At the completion of your internship experience, your Site Supervisor is asked to complete a formal evaluation. What follows is the instrument provided for this evaluation. It is your responsibility to provide the Site Supervisor with a copy of this instrument and to schedule an evaluation review meeting prior to your final review with your University Supervisor.

The process recommended for this evaluation is as follows:

1) Provide the Site Supervisor with a copy of the evaluation form well in advance of your scheduled review session.

2) Meet with your Site Supervisor to review his/her evaluation.

3) Both you and the Site Supervisor should sign the evaluation form.

4) A copy of the signed evaluation form should be submitted to your University Supervisor prior to your final review session.
