A request for the use of Marywood University facilities is not considered official until this form is completed and signed by a representative of the organization requesting the use of the facility. The use of a room is not guaranteed unless a confirmation form is received from the scheduling office. A signature on this request form signifies that the "Policies and Procedures for the Use of Campus Facilities" brochure was referred to and understood. The University is not responsible for conflicts resulting from failure to comply with this procedure.
